Optimise array controller addObject: by eliminating entirely pointless rearrange when added objects did not pass the filter. Also avoided double rearrange if clearsFilterPredicateOnInsertion.
...
CPArrayControllerPerformance.j execution time on "testAddObject_, sorted, filtered" down from ~420ms to ~60ms on my machine.

- Fixed a bug where the right control was incorrectly recreated with a default value when switching operators.
...
- As per cocoa, the predicate is recomputed when you send the right control action, not when you change text. Update-as-you-type can be turned ON by subclassing -_sendsActionOnIncompleteTextChange method. Default is NO.
- Rewrote _reconfigureSubviews method, should be faster.

This patch makes nib2cib dealing with NSButtons much more better by setting particular offsets

If there is no header view, there should be no corner view. This is what Cocoa does. Also for correctness -enclosingScrollView should be used to get the table's scrollview. Test app included.
