Alexander Ljungberg bdcc1539ac Use an ephemeral subview for the bar of a progress indicator. 
This is clearer code and avoids encoding the 'pill view' automatically.

Alexander Ljungberg da055f83b3 Fixed: CPProgressIndicator encoder wasted space and caused errors. 
The errors would cause exceptions such as "Not allowed to load local resource: file:///<some local path from the machine that ran nib2cib>" when loading cib driven app through a web server.

Alexander Ljungberg c16c0309c8 Fixed: CPOutlineView setMenu: was not effective. 
A CPOutlineView without a delegate `outlineView:menuForTableColumn:item` method, would not display its context menu as specified by `setMenu:` or in IB.

Alexander Ljungberg a0c7305334 Fixed: collection views cleared their selection whenever the content was changed. 
This was unlike Cocoa and prevented CPArrayController's selectsInsertedObjects from working.
