Removes the bitcoind RPC config entirely, so lndhub only uses LND's transaction monitoring. The bitcoind integration is broken as is.

Tested/running on bitcoin-2. My balance is now correct for an account that topped up previously via on-chain address, as well as for the jimmy test user, who now has a few sats to play with.
